Referrals
Referrals are required with an HMO insurance. Referrals need to be requested by the patient and should be done at least 1 week before your appointment to give your primary care physician (PCP) enough time to complete. Once it has been issued electronically, we will be able to obtain it for you.

Please note, that if your referral is not in the system upon your arrival for your appointment, it will be your responsibility to follow up with your PCP. Without a referral, UHA will not be paid. Therefore, the patient will be responsible for all costs of the visit and including any testing necessary to complete your visit.
